Job Summary

Seeking experienced RN Care Managers for a 13-week assignment. Positions are full-time (40 hours per week) with variable shifts: either 8:30 AM to 4:30 PM or four 10-hour shifts from 7:30 AM to 5:00 PM. No on-call duties, but weekend and holiday rotation is required.

The RN Care Manager will coordinate concurrent utilization management activities hospital-wide and serve as the primary liaison between patients, healthcare teams, the organization, and payors during hospital stays.

Key Responsibilities

• Conduct assessments of patient needs and identify resources for transitioning patients to the next care setting.
• Collaborate, educate, and lead interdisciplinary teams during daily care rounds, care conferences, and care planning.
• Communicate effectively in written, verbal, and electronic formats.
• Work with community organizations to secure durable medical equipment, health insurance authorizations, and arrange transportation.
• Participate in managing the length of stay for inpatient hospital populations.
• Understand hospital reimbursement methods and quality measures relevant to care management.

Required Qualifications

• Registered Nurse (RN) license required.
• Associate or Bachelor’s degree in Nursing.
• 5 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review (highest required years of experience).
• Experience in utilization review and discharge planning is mandatory.
• Active Basic Life Support (BLS) certification required.
• Certification by a nationally recognized case management or utilization review organization preferred (e.g., CCM or ACM).

• Weekend and holiday rotation required.
• Attention to detail required for timecard submission to ensure proper processing.
