Job Opportunity: RN Hospice Case Manager

Location: Barre, Vermont
Employment Type: Contract
Contract Length: 20 weeks

Job Description

Seeking an experienced Hospice RN for a hospice home care agency. The role involves hospice case management and providing hospice care in a home setting.

Required Qualifications

Registered Nurse (RN) license
Experience with hospice case management
Experience providing hospice care in a home setting
Prior travel experience is preferred but not explicitly required
Experience with McKesson/Netsmart EMR preferred

Work Schedule

Friday through Tuesday (open to Saturday through Tuesday)

Additional Notes

Preference for nurses with no Return to Office (RTO) requests for this contract
Prior experience with rural travel preferred
