Job Description
Join as a Home Health Occupational Therapist providing in-home care to patients recovering from illness, injury, or hospitalization. Empower patients to regain independence and improve daily function through personalized therapy interventions.
Key Responsibilities
- Perform in-home assessments to identify functional impairments and therapy needs
- Develop and implement individualized plans of care focusing on safety, self-care, and mobility
- Provide hands-on therapy interventions targeting activities of daily living (ADLs), home safety, and patient-specific goals
- Educate patients and caregivers on adaptive equipment and techniques
- Collaborate with physical therapists, nurses, aides, and physicians for coordinated care
- Document patient progress and adjust care plans as needed
- Support safe patient transitions and promote long-term functional wellness
Required Qualifications
- Licensed Occupational Therapist (OTR/L) in New Mexico
- Minimum of 1 year of clinical experience (home health experience preferred)
- Strong problem-solving, communication, and time management skills
- Ability to work independently in patients’ homes
- Experience with EMR systems and mobile charting tools
- Passion for enabling patient independence and dignity through skilled therapy
