LPN/LVN - Rehab (Travel Position)

Location: Madison, Wisconsin
Contract Length: 13 weeks

Job Description

• Work in a 49-bed acute inpatient rehabilitation facility specializing in stroke, brain and spinal cord injuries, amputations, complex orthopedic injuries, and other conditions requiring inpatient rehab.
• Patient population consists of adults with high acuity needs, often referred from a Trauma Center.
• The unit functions similarly to a medical-surgical unit with high acuity patients.
• Patient ratio is approximately 1:7.
• Equipment includes mobility aids (ceiling lifts, Sara Stedy, Sara Plus, slide boards), vital machines, EKG, blood glucose monitors, and EPIC EHR for charting.
• Nurses are expected to assist CNAs and respond to call lights to ensure patient safety.
• Call requirements apply.
• Weekend and holiday rotations every other weekend.
• Shift: 12-hour shifts (6:30 PM to 7:00 AM).
• Float may be required in the near future to a satellite inpatient rehab facility within 20 minutes.

Required Qualifications

• Valid LPN / LVN license in Wisconsin
• Minimum 1 year of LPN / LVN experience within the last year
• Experience in medical-surgical nursing
• Experience with EPIC electronic health record system
• BLS certification

Additional Information

• Orientation dates available throughout 2026.
• Strict attendance and tardy policies apply.
• Mandatory 30-minute unpaid breaks per shift unless otherwise approved; two 15-minute paid breaks included in 12-hour shifts.
• Staff must clock out for breaks and cannot stay past shift end without supervisor approval.
• Local candidates are not accepted for this travel position.

This role is ideal for LPNs/LVNs with solid inpatient rehab or med-surg experience, comfortable working in a high-acuity rehab setting, and proficient with EPIC.
