LPN/LVN - Rehab (Travel Position)
Location: Madison, Wisconsin
Contract Length: 13 weeks
Job Description
- Work in a 49-bed acute inpatient rehabilitation facility specializing in stroke, brain and spinal cord injuries, amputations, complex orthopedic injuries, and other conditions requiring inpatient rehab.
- Patient population consists of adults with high acuity needs, often referred from a Trauma Center.
- The unit functions similarly to a medical-surgical unit with high acuity patients.
- Patient ratio is approximately 1:7.
- Equipment includes mobility aids (ceiling lifts, Sara Stedy, Sara Plus, slide boards), vital machines, EKG, blood glucose monitors, and EPIC EHR for charting.
- Nurses are expected to assist CNAs and respond to call lights to ensure patient safety.
- Call requirements apply.
- Weekend and holiday rotations every other weekend.
- Shift: 12-hour shifts (6:30 PM to 7:00 AM).
- Float may be required in the near future to a satellite inpatient rehab facility within 20 minutes.
Required Qualifications
- Valid LPN / LVN license in Wisconsin
- Minimum 1 year of LPN / LVN experience within the last year
- Experience in medical-surgical nursing
- Experience with EPIC electronic health record system
- BLS certification
Additional Information
- Orientation dates available throughout 2026.
- Strict attendance and tardy policies apply.
- Mandatory 30-minute unpaid breaks per shift unless otherwise approved; two 15-minute paid breaks included in 12-hour shifts.
- Staff must clock out for breaks and cannot stay past shift end without supervisor approval.
- Local candidates are not accepted for this travel position.
This role is ideal for LPNs/LVNs with solid inpatient rehab or med-surg experience, comfortable working in a high-acuity rehab setting, and proficient with EPIC.
