Employment Type: Long-Term Assignment (LTA)

Job Details

• Start Date: January 5 (Non-negotiable)
• Shift Options: 36 or 48 hours per week
• Floating: Required to float between units or facilities within a 35-mile radius based on staffing needs, patient census, and acuity. Notification will be given before scheduled shifts if floating is required.

Required Credentials & Compliance for Onboarding

Licenses & Certifications

• Current nursing license with Nursys verification within 30 days prior to start
• BLS certification (AHA required)
• NIH Certification (Group A updated yearly; Group B-F every 2 years)

Testing & Exams

• Ascension Medical Surgical Exam (minimum 80%, max 2 attempts)
• Ascension EKG Exam (minimum 80%, max 2 attempts; updated edition as of Dec 2023)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, max 2 attempts)
• Ascension Cerner Test-Out (Cerner test results and training checklist with signed attestation)
• Ascension Skills Checklist (clinical only; completed within 1 year of start, updated annually)

Health & Immunizations

• Influenza Vaccine or approved exemption (mask required if exempt during flu season Oct 1 - Mar 31)
• MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
• Tdap: Valid within 10 years (no declinations accepted)
• Hepatitis B: Vaccination series completion or signed declination
• Fit Test: Required for all travelers; preferred before start, can be completed onsite
• 2 Step PPD or IGRA (Quantiferon/Tspot) with annual TB questionnaire
• Positive PPD requires negative chest X-ray within 1 year and TB questionnaire
• Health Statement or physical exam prior to first assignment, updated annually
• Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually

Professional Requirements

• Ascension Resume/Profile
• Two professional references prior to first assignment
• Criminal Background Check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry (updated annually or after >90 days break)
• EPLS check within 90 days of start (rerun if >90 days break)
• Education Verification relevant to clinical work within 90 days of start
• Employment Verification for past 7 years (updated annually or after >90 days break)
• Confirmation of Assignment documentation upon initial assignment and each extension
• Driver’s License/State ID (valid, matching address requirements)
• Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)
• Ascension Nursing Contract Worker Onboarding (completed onsite)
• Ascension Nova StatStrip pre-hire modules (required for non-allied positions)
• Ascension Badge Photo (forward-facing headshot with white or blue background, no filters)
• Ascension Color Vision test (completed prior to start, updated every 3 years)

Additional Notes

• Floating assignments are based on staffing needs and will be communicated in advance.
• Some optional documents may be requested by unit leaders during onboarding.
• All exams and certifications have specific passing score requirements and attempt limits.
• Compliance with all health and professional requirements is mandatory for clearance and continued assignment.
