Job Description

Seeking a Surgical Assistant with certification as RNFA, PA, or SA (certification only; CST not accepted) to work at a Level 3 trauma center.

• Shift: Monday to Friday, 07:00 AM to 5:30 PM
• On-call: 1 weekday per week and 1-2 weekend days per month

Required Qualifications

• Valid RN license in Ohio
• Certification as RNFA, PA, or Surgical Assistant (SA)
• Minimum of 1 year of First Assist experience, including experience within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Operating Room (OR) experience
• Trauma experience
• Bariatric surgery experience
• Orthopedics experience
• Ortho Trauma experience
• Thoracic Surgery experience
• Vascular Surgery experience

Required Onboarding Credentials and Compliance

• Professional license and/or certification primary source verification within 30 days of start; updated upon expiration.
• COVID-19 vaccine preferred but not required; medical or religious exemptions accepted.
• Immunizations and health requirements:

  • MMR: Positive titers within last 15 years or booster protocol as specified.
  • Varicella: Positive titers within last 15 years or booster protocol as specified.
  • Hepatitis B: Proof of full series, partial series with declination, or positive titers within 15 years.
  • Tdap: Vaccination within last 10 years (no declinations accepted).
  • Influenza vaccine required during flu season (Oct-Mar) or mask and declination forms completed.
• Fit test for respirators completed within 1 year prior to start; updated annually.
• TB testing: 2-step PPD or IGRA within last 6 months; if positive, clear chest x-ray and current TB questionnaire required.
• Physical exam within 1 year of start confirming ability to work without restrictions.
• Drug screen within 60 days of start (10-panel, lab report required; instant screens not accepted).
• Criminal background check within 6 months of start, including social security trace, county searches, national sex offender registry, and OIG exclusion check.
• Fingerprint background checks as per Ohio residency duration (BCI&I and FBI checks).
• Employment and education verification through third-party.
• Skills checklist, competency exam (minimum 80%), and core competencies (minimum 80%) completed within 1 year prior to start.
• Completion of required safety and compliance training modules prior to clearance; renewal annually.
• Non-expired photo ID (Driver’s License or State ID).
• Badge photo (headshot, shoulders up, plain background, JPEG).

Additional Notes

• Attestations are generally not accepted except where explicitly allowed (e.g., fingerprint attestation pending results).
• Some requirements are one-time upon initial hire and not annual (e.g., skills checklist, competency exam, core competencies, OIG search).
• Masking and vaccination declination forms are specific and must be completed if applicable.
