Job Opportunity: Home Health Registered Nurse

Location: Milwaukee, Wisconsin
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Specialty: Home Health Nursing
  • Minimum Experience Required: 1 year recent experience in specialty
  • Prior travel experience: Not accepted for first-timers (i.e., prior travel experience required)
  • Average Patients per Day: 5-7 (to be confirmed during interview)
  • Guaranteed Hours: 40 hours per week
  • Types of Cases: Geriatric patients
  • Territory: Up to a 60-mile radius from the office/hub
  • Locals accepted at travel rate
  • On-call required (details to be discussed during interview)
  • Charting must be completed by 9 AM the following morning
  • Timely communication is required
  • Dress Code: Scrubs (any color), no jeans
  • Cell phone use: Not allowed; iPad provided
  • Mileage reimbursed at IRS standard rate
  • Flexibility with territory expected

Required Qualifications

  • Valid Registered Nurse (RN) license in Wisconsin
  • Minimum 1 year recent experience in Home Health nursing specialty
  • Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Experience with OASIS documentation (required)
  • Experience with Homecare Homebase (HCHB) electronic health record system (required)
  • Prior travel experience required (first-time travelers not accepted)

Compliance and Background Requirements

  • Background checks required within 90 days of start, including:
    • SSN Trace
    • County searches for last 7 years of residence/work/school
    • FACIS Level 1
    • Nationwide Sex Offender Registry
    • Motor Vehicle Records (MVR)
    • Education and Work History Verification
    • State-specific searches as applicable
  • EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
  • Fingerprinting may be required depending on state placement (updated annually)
  • Education verification of highest degree related to profession
  • NURSYS verification for RN license within 90 days of start
  • Medication/Pharmacology competency exam as applicable
  • Signed Orientation Safety Manual Acknowledgement (updated annually)
  • Personal or Auto Insurance proof required if driving is part of job duties
  • Facility-specific policies and attestations must be completed prior to start

Health and Immunization Requirements

  • Physical exam within last 12 months (not required annually)
  • Negative TB test or chest X-ray within required timeframes
  • Annual TB questionnaire required
  • Immunizations or titers for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug screening (9-panel) within last 12 months
  • Seasonal flu vaccination or facility-specific declination form during flu season
  • COVID vaccination or facility-specific declination

Additional Requirements

  • 2 professional references within 2 years of start (travel, allied, per diem acute care)
  • Full 7 years of work history documented with no gaps
  • Annual skills checklist
  • Work history verification for 7 years within 90 days of start

Notes

  • First-time travel nurses are not accepted for this position.
  • Flexibility with travel territory and on-call availability is expected.
  • All charting and communication must be timely and meet facility standards.
  • Mileage reimbursement is provided.
  • Dress code is scrubs only; no jeans allowed.
  • Cell phone use during work is prohibited; iPad will be provided for documentation.
