Job Details

- Weekend Coverage: Every other weekend
- Float Requirement: Yes, as needed by facility
- Local Candidates: Must live more than 75 miles away
- Electronic Health Record: EPIC required
Required Experience & Skills
- Valid LPN / LVN license in New York
- Minimum 2 years of psychiatric nursing experience, including experience within the last year
- Long Term Care experience
- Med Surg experience
- Behavioral Health experience
- Proficient in behavior management techniques
- Skilled in managing assaultive behavior
- Knowledgeable in de-escalation protocols
Required Certifications & Licenses
- BLS issued by American Heart Association
Compliance & Background Requirements
- Government-issued photo ID required
- 7-year criminal background check within 3 months of start (including SSN trace, national criminal, sex offender, education verification)
- EPLS/GSA/SAM and HHS/OIG checks within 3 months of start, run monthly
- Foreign equivalency education evaluation if applicable
- Education verification from primary source required
- Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
- Fingerprinting and disciplinary action searches may be required depending on unit assignment
- New York State Nurse Aide Registry registration required within 3 months for long-term care settings
- Sex offender registry search within 30 days of start
- Medicaid exclusion list check within 3 months of start, run monthly
Health & Immunization Requirements
Candidates must complete health-related requirements and lab work within 5 days of signing the contract:
- Physical exam within 1 year, updated annually
- Fit mask test within 1 year, updated annually (specific mask types accepted)
- Negative TB test or chest X-ray within 1 year
- TB questionnaire annually (acceptable in lieu of annual TB test)
- Immunizations or positive titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (surface antibody positive titer, completed series, or declination)
- Varicella
- Seasonal flu vaccination or signed declination during flu season
- Snellen eye exam within 1 year (not required annually)
- COVID vaccination copy preferred but not required
Additional Requirements
- Orientation safety manual acknowledgement required
- Medication test/pharmacology competency exam as required by profession
- Color blind testing required once
- Competency exams as required by specialty
- Clear, passport-style photo for badge
- Employment application and work history verification required
- References required for some candidates (2 references within 3 years)
