Job Opportunity: Licensed Vocational Nurse (LVN) – Primary Care

Location

• Claremont, California
• Must live within 30 miles of the work site

Employment Type

• Travel/Contract
• Contract length: 13 weeks

Schedule

• 5 shifts per week
• Day shift: 8:00 AM to 5:00 PM
• No weekends, on-call, or holiday shifts

Job Summary

• Support the Practitioner in delivering patient care and managing patient flow
• Administer medications and assist with clinical duties similar to Medical Assistant responsibilities
• Follow physician orders including discharge instructions
• Provide patient education
• Manage medication inventory
• Train clinical staff on policies and procedures
• Manage daily Back Office staff schedules
• Other duties as assigned

Required Qualifications

• Valid LPN/LVN license in California
• Minimum 1 year of Long Term Care experience within the last year
• BLS certification issued by the American Heart Association
• IV Therapy certification
• Phlebotomy certification
• High school diploma or equivalent
• Family Practice experience

Dress Code and Appearance Standards

• No facial hair allowed
• No acrylic nails or unnatural hair color
• Tattoos must be covered
• No visible body piercings including facial or nose rings (except earrings limited to earlobes; no hoop or extended earrings)

Required Background and Compliance Checks

• Background check covering the last 7 years, including Social Security address check (last 15 years), national criminal search, sex offender search, education verification, and international searches if applicable
• EPLS/GSA/SAM and HHS/OIG checks within 30 days prior to start
• Medi-Cal Exclusion Search within 30 days prior to start (California-specific)
• Verification of education and licensure within 30 days prior to start

Health and Immunization Requirements

• Physical exam within 12 months prior to start, cleared to work without restrictions
• Negative TB test or chest X-ray per facility protocol
• Tdap vaccine or declination within 10 years prior to start
• Immunity or vaccination records for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and Hepatitis C as per facility requirements
• Negative 10-panel drug screen within 30 days prior to start; annual drug screen required for travel and per diem staff
• Fit mask test with 3M Aura 1870+ N95 mask (universal size) required; onsite fit testing available if approved
• Seasonal flu vaccination or facility-specific declination required annually

Required Documentation and Training

• Two professional references, including one from most recent experience
• Complete 15 years of work history on application, including locations and any time off
• Annual skills checklist and competency exams (medication/pharmacology competency with minimum 80% score)
• Completion of facility-specific orientation packet, training modules, and policies prior to start
• Photo for badge on plain blue background without facial obstructions or visible tattoos/piercings
• Signed facility-specific acknowledgments including privacy, security, timekeeping, and job description
• COVID-19 and flu vaccination declination forms if applicable

Additional Notes

• IV certification required for all LVNs at this facility
• Management of Assaultive Behavior (MAB) training not required for this role
• No weekends, on-call, or holiday shifts expected
• Strict adherence to facility policies and professional appearance standards is mandatory
