Physical Therapist - Home Health (Travel/Contract)
Location: Walla Walla, Washington
Contract Length: 13 weeks
Job Details
- Schedule: Monday to Friday, Day shift (8:00 AM - 5:30 PM), no weekends
- Hours: 40 hours per week, guaranteed 30 patient visits weekly
- Patient Load: Approximately 5-7 patients per day (to be confirmed during interview)
- Caseload: Geriatric patients
- Territory: Up to a 60-mile radius covering parts of Walla Walla, Columbia, and Umatilla counties
- Dress Code: Scrubs of any color; jeans are not allowed
- Technology Provided: iPad provided; cell phone use not allowed during work
- Mileage: Reimbursed at IRS standard rate
- Documentation: All charting must be completed by 9 AM the following morning
- Communication: Timely documentation and communication required
Required Qualifications
- Valid Physical Therapist license in Washington
- Minimum 1 year of Physical Therapist experience within the last year
- Basic Life Support (BLS) certification
- Experience with Homecare Homebase (HCHB) EMR
- Experience with OASIS documentation
Compliance & Background Requirements
- Background checks must be current within 90 days and include:
- SSN Trace
- County searches for last 7 years of residence/work/school
- FACIS Level 1M
- Nationwide Sex Offender Registry
- Motor Vehicle Records (MVR)
- Education Verification
- Work History Verification (7 years)
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting required if candidate has not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
- Employment application must include full 7 years of work history with no gaps
Health & Safety Requirements
- Physical exam within the last 12 months (not required annually)
- Fit test for N95 mask (no specific model required)
- Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if previously positive
- Annual TB questionnaire required
- Immunizations or positive titers required for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within 12 months (instant acceptable)
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID vaccination or facility-specific declination required
Additional Requirements
- Signed Orientation Safety Manual Acknowledgement (updated annually)
- Proof of current auto insurance required due to driving responsibilities
- Completion of facility-specific Acceptable Use Policy prior to start
- Two professional references within 2 years of start required
