Job Description

• Provide home health physical therapy services across multiple nearby areas (including Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek, Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, Bay Point).
• Shift: Day shift, 8 hours per day (7:00 AM - 4:00 PM), 5 days per week.
• Expected to see 5-6 patients per day with a 1:1 patient ratio.
• Guaranteed 36 hours weekly (note: guaranteed hours do not apply during the first 2 weeks for ramp-up, holidays, or time off).
• Weekend patient visits are optional to meet productivity goals.
• No on-call requirement.
• Own transportation is required; occasional travel radius may exceed 50 miles.
• Documentation and timely communication are required; documentation tools will be provided and must be returned.
• Charting systems used: MediScrib & OASIS (training provided).
• Dress code: Any color scrubs.
• Parking is free.
• Mileage reimbursement at IRS standard rates.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum 1 year of Physical Therapist experience, including experience within the last year.
• Experience in a Home Health setting.
• Current Basic Life Support (BLS) certification.

Additional Requirements & Compliance Documents

• Government-issued photo ID (Driver’s License/State ID).
• Emergency Contact Form.
• I9 Attestation form.
• Background checks including 7-year history, EPLS/GSA/SAM, and HHS/OIG checks (updated annually).
• Advanced certifications such as ACLS and PALS are noted but not explicitly required for this role.
• Auto insurance and current vehicle registration (must be maintained throughout assignment).
• Physical exam within 1 year prior to start, confirming clearance to work without restrictions.
• Immunizations and health screenings required:

  • Tdap vaccine within 10 years or annual declination.
  • Negative TB test or chest X-ray within 1 year.
  • Measles, Mumps, Rubella (MMR) vaccination or positive titers within required timeframes.
  • Hepatitis B titer or vaccination series or declination.
  • Varicella vaccination or positive titer.
  • 10-panel drug screen within 1 year.
  • Seasonal flu vaccine or declination (required year-round).
  • COVID-19 vaccination documentation or facility-specific declination form.
• Professional references:

  • 2 professional references for travel, allied, or per diem acute care roles.
  • 1 professional reference for per diem non-acute care roles.
• Work history verification: 1 year of employment related to the profession.
• Annual skills checklist and competency exams as applicable.

• First-time travelers are not accepted for this position.

• The system attempts to group clients to minimize driving time.
• Timely and accurate documentation is essential.
