Job Opportunity: Registered Nurse - Home Health
Location: Cordova, Tennessee
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
- Travel required up to 75 miles from home clinic; approximately 150-200 miles weekly.
- Schedule: Monday to Friday, 8 AM to 5 PM.
- Visits include Assisted Living Facilities and Patients' Homes.
- On-call required approximately 1-2 days per week (varies by location).
- Local candidates accepted at the same rate.
- Must have reliable transportation, current driver's license, and agency-required liability insurance.
Requirements
- Specialty: Registered Nurse (Home Health)
- Experience: Minimum 1 full year of Adult Home Health experience OR 1 full year Hospice experience (whichever is higher).
- Prior Travel Experience: Not required.
- First-timers: Not accepted; must have recent specialty experience.
- Certifications: Current CPR and BLS required.
- Licensure: Must have license in hand; pending licenses not accepted.
- EMR Systems: Proficiency with HomeCare HomeBase (HCHB) within the last 6 months required; RNs must also be proficient in OASIS. Retraining required if last use was over 6 months ago.
- Dress Code: Scrubs.
Productivity Expectations
- Weekly productivity: 32-36 points (points assigned based on visits and activities).
Compliance & Documentation Requirements
- Valid Driver’s License/State ID.
- Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
- Fingerprinting required depending on state placement.
- Education verification for nursing degree.
- Health documents including:
- Physical exam within 12 months.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
- Drug testing (10-panel).
- Flu vaccination or declination documentation.
- COVID vaccination series or declination.
- References: Two references required per Aya standards.
- Work history verification: At least two most recent employers or last 7 years.
- Completion of facility-specific compliance documents and training prior to start, including but not limited to:
- Code of Conduct
- HIPAA and Privacy Policy
- Compliance Training Certification
- Home Health Scenarios and Core Policy Acknowledgement
- Confidentiality and Privacy forms
- COVID-19 Policy and Attestation
- Various state-specific forms and attestations as applicable.
Additional Notes
- No guaranteed hours; if patient census is low, contractor may be floated to nearby clinics to pick up hours.
- Must be willing to travel within the assigned territory.
- On-call and weekend requirements vary by location and should be discussed during the interview.
This position is a contract assignment only with no option for permanent hire at the end of the contract.
