Job Details

- Block Scheduling: Not accommodated
- Local Radius Rule: 75 miles (locals not accepted)
- Pending License: Accepted if obtained before start
- Guaranteed Hours: Based on signed contract hours
Key Requirements
- Must be comfortable working with a high patient ratio (1:44)
- Must have LTC experience
- BLS certification required (AHA or RQI acceptable)
- Physical ability to meet job demands (physical exam within 1 year)
- Must comply with attendance policy: missing more than two shifts in a 13-week contract without reasonable explanation may lead to contract cancellation
Certifications & Licenses
- Required:
- BLS (Basic Life Support)
- State License (Primary Source Verification)
- NURSYS Verification (for RNs, if applicable)
- Orientation Safety Manual Acknowledgement
- Preferred/As Applicable:
- ACLS (Advanced Cardiac Life Support) preferred for RNs
- Other certifications (PALS, NRP, TNCC, EFM, NIH Stroke Certification, STABLE) may be required depending on unit
- Pass/Fail Testing (EKG/Medication) required for RNs only
- Medication Test/Pharmacology Competency Exam required annually
- Competency Exam required annually
Background Checks & Verifications
- 7-year criminal background check within 1 year of start, updated annually
- South Carolina Statewide Search required regardless of residence history
- Education verification required:
- CNAs/LVNs: High School Diploma/GED verification
- RNs and Allied/Non-Clinical: Highest level of education verified (associates or higher)
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for renewals/extensions
Health & Immunization Requirements
- Physical Exam within 1 year, updated annually
- Fit Mask test within 1 year (N-95 BYD, Duckbill, or 3M-1860 masks)
- Negative TB test or Chest X-Ray within 3 months of start (Qgold, Tspot preferred)
- Immunizations or proof of immunity/exemption required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (vaccine series, positive titer, or declination)
- Varicella
- Drug Testing within 60 days of start (10 panel + Oxy + Marijuana)
- Seasonal Flu Vaccination or declination required during flu season (11/01 - 3/31)
- COVID-19 vaccination (Dose 1 and 2) required or declination form
Additional Requirements
- Government-issued photo ID (Driver’s License/State ID) required and must be carried during work hours
- Social Security Card required if used on I9 form
- Employment application required
- Skills checklist updated annually for applicable units
- Photo for badge (headshot on plain background)
- Facility-specific documents and compliance checklists as required
Notes
- All Return to Office (RTO) requests must be included in the initial submission; late requests will not be considered.
- Candidates must pass required exams and competencies as outlined.
- Attendance and compliance with facility policies are strictly enforced.
This position is ideal for LVNs/LPNs with LTC experience who are comfortable managing a high patient load in a long-term care setting and meet the outlined certification, background, and health requirements.
