Job Description

Position: Inpatient Pharmacist
Work Hours: Weeknights 9pm-7am, Weekend nights 7pm-7am
Requirements:
- PharmD degree
- Licensed to practice pharmacy in North Carolina

Required Credentials & Documentation

Licensure & Certifications

Pharmacy license valid in North Carolina
ACLS (Advanced Cardiac Life Support) - ARC, AHA, or US Department of Defense approved
BLS (Basic Life Support) - ARC, AHA, or US Department of Defense approved
Other certifications as applicable (e.g., PALS, NRP) depending on facility/unit requirements

Background Checks

7-year criminal background check within one year of start, updated annually
Must include all locations lived/worked in past 7 years
EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for renewals
Completion of Criminal History Investigation Form if background check returns a hit

Health & Immunization Requirements

Initial physical exam, updated annually
Negative TB test or chest X-ray within 1 year, updated annually
Immunizations or titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
Flu vaccination or approved medical/religious exemption required during flu season (Oct 1 - Apr 30)
9-panel drug screen within 90 days of start, updated annually

Competency & Testing

Medication Test/Pharmacology Competency Exam as applicable
Core competency exams required
Facility-specific orientation and post-tests must be completed

Facility-Specific Requirements

Passport-style photo for badge (shoulders up, front facing, plain background) submitted before start
Completion of self-study orientation and accountability statements
Submission of personal data sheet
Compliance with facility-specific policies and documentation

Additional Notes

Proof of 2 clinical references required
Performance evaluations required if previously worked with the system over 1 year
Employment application/resume must be complete
Skills checklist updated annually for applicable units
