Required Qualifications

• License: Must hold a valid LCSW license for Oregon.
• Experience: Minimum 1 year of experience in the specialty (Social Services / LCSW).
• Prior Travel Experience: Not explicitly required.

Certifications (as applicable to LCSW)

• Basic Life Support (BLS) - AHA or Canadian Heart & Stroke Foundation certification required.
• Advanced Cardiac Life Support (ACLS) - May be required; verify with clinical track.
• Other certifications (PALS, NRP, CPI, etc.) generally not required for Social Services but verify if specified.

Compliance and Background Requirements

• Background Check within 45 days of start, covering all locations lived/worked in the past 7 years.
• Education Verification required.
• Drug Screening: 10 Panel Drug Screen within 90 days of start (no instant tests).
• Physical Exam within 1 year of start, signed by MD, NP, PA, or DO.
• Immunizations and Health Documents:

  • Tetanus/Diphtheria/Tdap (within 10 years or declination accepted)
  • Negative TB test or Chest X-Ray within 12 months, updated annually
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B vaccination or signed declination
  • Varicella vaccination or positive titer
  • Seasonal Flu Vaccine or declination (required during flu season)
  • COVID vaccine or declination
• Respiratory Safety Clearance required for travel candidates (annual).

Facility Specific Requirements

• Must complete Providence Health and Services annual regulatory and specialty-specific WorkRamp training modules prior to start.
• Badge photo required (passport style with neutral background).
• May be required to float to other Providence facilities within a reasonable distance (float policy acknowledgment).
• Epic training modules required for charting system access.

References and Employment History

• Minimum of 2 references covering 1 year of employment within the last 3 years in a comparable specialty/setting.
• Employment application covering at least the last 5 years required.

Additional Notes

• No on-call or block schedule requirements.
• Weekend work required once every two weeks.
• No floating between units.
• Charting system is Epic.
