- Two supervisory references within 1 year of start
- Previous 5 years of employment history verification (attestation accepted if necessary)
- Agency application/resume completed within 30 days of start
Health & Immunization Requirements
- Physical exam within 1 year of start, including Latex Screen
- Fit test for masks within 1 year of start
- Tetanus/Diphtheria/Tdap (required for units working with children; not required for ER unless Peds ER)
- Negative TB test or chest X-ray with annual updates as applicable
- TB questionnaire if positive TB result
- Immunizations or positive titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (declination accepted for low/equivocal titer)
- Varicella
- Seasonal flu vaccination or medical/religious exemption required during flu season
- COVID vaccine not required for this location; exemptions accepted if required elsewhere
Facility Specific Requirements
- Passport/badge style photo with solid white/beige background required
- Candidate must sign job description and confidentiality agreement prior to start
- Completion of standards of conduct and employee confidentiality statement prior to start
- Badge ID application must be completed prior to start
- Portfolio Epic Training Test Out form may be required depending on Epic experience
Additional Notes
- Medication, pharmacology, competency, age-specific, dysrhythmia/EKG, and color blindness competency exams may be required depending on profession/unit
- Some certifications and documents are unit-specific; refer to job description for applicability
This position is ideal for a Registered Nurse with emergency department experience who holds current ACLS, BLS, PALS, CPI, and NIH Stroke certifications, and is prepared to work every other weekend and holidays with possible floating duties.
