Required Qualifications
- Valid Interventional Radiology Technologist license in Utah
- Interventional Radiology Technologist experience within the last year
- 1 year of experience as an Interventional Radiology Technologist
- ACLS issued by American Heart Association
- BLS issued by American Heart Association
- PALS issued by American Heart Association
Compliance and Health Requirements
- Background check
- EPLS/GSA/SAM
- HHS/OIG
- Physical exam within 1 year of start
- Tdap within 10 years of start
- Negative TB test or chest X-ray
- TB questionnaire if there is a history of past positive TB
- Measles, mumps, and rubella immunity via 2 MMR vaccines or positive titer
- Hepatitis B surface antibody titer showing immunity
- Varicella immunity via 2 vaccines or positive titer
- 11-panel drug screen including MDMA within 30 days of start
- Flu vaccination or approved exemption
- Respirator medical clearance if respirator use is required
- Annual skills checklist
Additional Notes
- Time off during the contract should be communicated in advance
- Interventional Radiology-specific document is required
