Job Summary
The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to their families. Responsibilities include managing an interdisciplinary team, case managing assigned patient clinical records, and ensuring the highest quality of care in accordance with the Plan of Care. The RN will perform initial and ongoing patient assessments, develop and implement hospice care plans, and collaborate closely with the interdisciplinary team to provide comprehensive physical, psychosocial, educational, emotional, and spiritual support, including bereavement services.
Additional duties include:
- Reporting changes in patient condition to physicians
- Maintaining accurate and updated medical records
- Practicing infection control and standard precautions
- Participating in quality assessment and performance improvement programs
- Possibly participating in on-call schedules
- Supervising and supporting team members
- Educating patients, families, and caregivers on care needs and hospice philosophy
Contract Length
Approximately 14 weeks
Required Qualifications
Experience
- Minimum of 1 year hospice experience within the last year
- Experience in acute care, oncology, long-term care, and home health settings
- Knowledge of regulatory guidelines including Medicare, Medicaid, and ACHC
- Experience with Homecare Homebase software
Education
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Certifications & Licensure
- Valid Registered Nurse (RN) license in South Carolina
- BLS certification issued by the American Heart Association
Additional Requirements
- Valid driver’s license and current automobile insurance
- Ability to operate nursing assessment tools and common office equipment
- Reliable transportation for work-related travel
