Job Summary

• Provide direct patient care in accordance with the Nurse Practice Act in Home Health and/or Hospice settings.
• Collaborate with healthcare team members to ensure safe, timely, and quality patient care.
• Perform physical assessments, IV/infusion therapy, nursing interventions, pain management, medication administration, specimen collection, nutritional support, patient teaching, and discharge planning.
• Utilize evidence-based nursing processes to meet patient care needs and adjust care plans as necessary.
• Consult with appropriate personnel for ethical, legal, or medical dilemmas.
• Perform other duties as assigned.

Contract Length

• 10 weeks

Required Skills & Experience

• Minimum 2 years of acute clinical nursing experience.
• Clinical home health care experience is required for Home Health roles.
• For Hospice roles, either 2 years of acute clinical nursing or home health experience is required.

Preferred Skills & Experience

• Hospice nursing experience is preferred.

Education Requirements

• Completion of an Associate's Degree in Nursing or higher.

Certifications & Licensure

• Current Registered Nurse (RN) license in the State of California.
• Current BLS certification prior to clinical rotation.
• Must have a valid driver’s license and insured, reliable automobile.

This position involves providing skilled nursing care in home health and hospice environments, requiring strong clinical experience and the ability to work independently while collaborating with a multidisciplinary team.
