Required Credentials and Onboarding Requirements

Licenses & Certifications

• BLS (American Heart Association or Red Cross)
• ACLS (American Heart Association or Red Cross)
• NIHSS
• Nursys License Verification

Exams & Testing

• Prime Basic Pharmacology Exam (minimum score 80%)
• Prime Specialty Exam (minimum score 80%)
• Prime Core Competencies Exam (minimum score 80%)
• Prime Skills Checklist (submission required)

Professional Requirements

• Prime Education Verification
• Driver's License, State ID, or Passport
• Prime OIG Check
• Prime SAM/EPLS Exclusion Search (must show visible date)
• Prime Resume (submission required)
• Two Professional References (submission required)
• Prime Criminal Background Check (within one year of start date)
• Prime Badge Photo (JPEG headshot with plain background)
• Prime Form I-9 (Employment eligibility verification)
• Prime Job Description (signed by employee)
• Essentials Onboarding Packet
• Onboarding Education Attestation (must be completed prior to start)
• Prime Agency Attestation (completed last after all credentials uploaded)

Personal Health History & Immunizations

• COVID-19 Vaccine: 2 vaccines plus booster or declination completed
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Hepatitis B (declinations not accepted at some locations)
• Influenza Vaccine (optional, but recommended during flu season)
• TB Test (within 1 year of start date)
• Annual TB Questionnaire
• Positive PPD with Negative Chest X-Ray (optional)
• Varicella: Vaccination or titer showing immunity
• Tdap (declinations not accepted at some locations)
• Physical/Health Statement: Annual, must state clear to work
• Fit Test: Annual respirator fit test, must be completed prior to start (no onsite testing in Illinois)
• Prime Drug Screening: Minimum 10-panel test including THC, within 60 days of start

Additional Training

• DCI/BVP (Workplace Violence Prevention) training required; can be completed via Relias or similar platforms

Notes

• All credentials and onboarding requirements must be completed and verified prior to start.
• Floating between units requires flexibility and adaptability.
• Fit testing must be uploaded before onboarding; no exceptions for Illinois locations.

This position is ideal for an RN with telemetry experience who is comfortable working in multiple units and meeting comprehensive credentialing and onboarding standards.
