Physical Therapist (PT) - Travel Assignment

Location: Port Huron, Michigan
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Details

• Full-time (1.0 FTE)
• Work hours: 8:00 AM - 4:30 PM, Monday through Friday with alternating weekends
• Must be licensed to practice Physical Therapy in Michigan
• Basic Life Support (BLS) certification required

Required Credentials & Compliance

• Licensure: Valid Physical Therapist license in Michigan
• Certifications:

  • BLS (Basic Life Support)
• Vaccinations / Immunizations:

  • COVID-19: 2 vaccines plus booster or completed declination
  • MMR: Proof of 2 vaccines or positive titer
  • Hepatitis B: Required (declinations not accepted)
  • Influenza Vaccine: Required during flu season (Nov 1 - Mar 31) or declination
  • Varicella: Vaccination or titer showing immunity
  • Tdap: Required (declinations not accepted in some locations)
• Testing:

  • TB Test within 1 year of start date
  • Drug Screening: Minimum 10-panel test including THC within 60 days of start
• Health:

  • Physical/Health Statement annually, must state clear to work
• Background & Verification:

  • National Criminal Background Check within 1 year of start date (some states require before start)
  • Prime OIG and SAM/EPLS exclusion checks
  • Driver’s License, State ID, or Passport required for submission
• Exams & Checklists:

  • Prime Core Competencies Exam (minimum 80%)
  • Prime Skills Checklist
  • Prime Specialty Exam (minimum 80%)
• Other:

  • Prime Agency Attestation
  • Prime Form I-9 (Employment eligibility verification)
  • Prime Job Description acknowledgment on first day
