Occupational Therapist (OT) – Home Health (Days)

Location: Fort Dodge, Iowa
Employment Type: Permanent
Shift: Monday – Friday, 8:00 AM – 4:30 PM (8 hours/day)

Job Summary

Provide occupational therapy services in patients’ homes, delivering personalized therapy plans to help patients regain independence and improve quality of life. Collaborate with physicians and care teams to perform assessments, develop treatment plans, and provide hands-on treatment, education, and support.

Key Responsibilities

• Conduct home visits to patients requiring occupational therapy services.
• Perform ongoing interdisciplinary assessments of patients.
• Evaluate patient functioning using appropriate tests.
• Develop and implement occupational therapy procedures and functional activities based on physician’s treatment plan and patient evaluation.
• Administer treatment plans including environmental modifications.
• Oversee use of treatment modalities, prosthetic devices, and therapeutic appliances.

Required Qualifications

Education

• Graduate of an occupational therapy education program accredited by ACOTE (Accreditation Council for Occupational Therapy Education) or its successor organizations.

Licensure & Certifications

• Licensed or regulated as an Occupational Therapist in the practicing state (if applicable).
• Valid driver’s license with automobile insurance as required by state/organization.
• Maintain valid Basic Life Support (BLS) Healthcare Provider Card with re-certification.
• Mandatory reporter for Child & Dependent Adult Abuse.

Experience

• Minimum of 1 year experience caring for a similar patient population.
