Job Description

• Provide and document nursing care for sick and injured patients in their homes.
• Promote, maintain, or restore health, prevent illness, and support rehabilitation.
• Collaborate with healthcare providers and community representatives to coordinate home care.
• Evaluate patient and family responses to nursing interventions.
• Function independently under general guidance, following unit-specific policies and protocols.
• Utilize time management, communication, planning, delegation, and problem-solving skills.
• Exercise independent judgment and critical thinking.
• Maintain emotional stability in emergency situations.
• Accept on-call responsibilities and be flexible with work hours.
• Drive in inclement weather as part of job duties.
• Maintain confidentiality and adhere to patient rights and customer service standards.
• Document clinical data accurately in verbal and written form.
• Requested time off during the assignment must be submitted and approved at profile upload; no more than 5 days allowed.

Required Qualifications

• Valid LPN / LVN license in Massachusetts.
• Minimum 1 year of LPN / LVN experience, including experience within the last year.
• Current BLS certification issued by the American Heart Association or American Red Cross.
• Experience in a Home Health setting.
• Valid Driver's License.

Additional Requirements for Submission

• Provide one supervisory signed reference from the past two years of employment.
• Include all requested time off (RTO) at time of profile submission; scheduling requests after offer/interview will not be approved and may result in offer rescindment.
