Contract Length: 13 weeks
Job Details
- Specialty: Therapy/Rehabilitation (Occupational Therapy)
- Start Date: ASAP (Compliance process takes about 3 weeks)
- Shift: 8-hour day shifts
- Patient Ratio: 1:1 (May visit up to 6 patients per day)
- Typical Caseload: 4-5 geriatric patients per day
- Weekend Requirement: None
- On-Call Requirement: None
- Locals Accepted: Yes
- Transportation: Must have own transportation
- Travel Radius: Up to 100 miles occasionally, with client grouping to minimize drive time
- Documentation: Timely communication and documentation required; tools provided
Experience & Licensing Requirements
- Minimum 1 year of recent skilled Occupational Therapy experience within the last 3 years
- Inpatient experience not accepted
- Must hold a valid Washington state OT license or compact license
- BCLS certification required
Compliance & Certification Requirements
- Certifications:
- BLS (Basic Life Support)
- HIPAA Safety Manual Acknowledgement (annual)
- State License Verification (within 90 days of start and upon expiration)
- NURSYS Verification (if applicable)
- Competency Exams: Medication/Pharmacology and general competency exams as applicable
- Background Checks & History:
- Background check within 90 days of start, updated annually
- EPLS/GSA/SAM checks within 90 days of start
- Employment application with full 7 years of work history, no gaps
- Work history verification (7 years)
- Education verification of highest degree related to profession
- Professional references (2 for travel/allied/per diem acute care; 1 for per diem non-acute care)
- State-specific criminal history and abuse registry checks as applicable
- Health Documents:
- Physical exam within last 12 months
- Negative TB test or chest X-ray within required timeframes
- Annual TB questionnaire (required for all candidates)
- Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
- Drug testing (9 panel) within last 12 months
- Seasonal flu vaccination or declination (2024-25)
- COVID-19 vaccination or medical/religious exemption (dose 1, fully vaccinated, booster)
Additional Notes
- The system groups clients to minimize windshield time and maximize productivity
- Documentation tools will be provided and must be returned at contract end
- Fingerprinting requirements apply for candidates who have not lived in Washington for the past 3 years (completed on-site)
