Overview

• Travel/Contract role in Hazard, Kentucky
• 34-bed Stepdown/PCU unit
• 1:5 ratios
• 13-week assignment
• Day shift: 3x12 or 4x12, 7:00–19:30
• 36 hours per week
• Float required within scope of practice
• Every other weekend required
• Blocked scheduling cannot be accommodated
• Locals not accepted; candidate must reside at least 75 miles from the facility

Required Qualifications

• Associate of Science in Nursing / Associate Degree in Nursing
• Valid RN license in Kentucky
• 2 years of Stepdown experience
• Stepdown experience within the last year
• AHA BLS
• AHA ACLS
• NIHSS
• Medication Test/Pharmacology Competency Exam
• Competency Exam within 1 year prior to start date, renewed annually
• Dysrhythmia/EKG Competency Exam within 1 year of start, updated annually
• Core Part I
• Core Part II
• Core Part III
• 2 references within 1 year of start date, with at least one from a supervisor or higher
• 7-year work history verification
• Valid driver’s license
• Flu vaccination or approved medical/religious exemption
• Negative TB or Chest X-Ray
• Physical exam or physician note within 1 year prior to start
• Tetanus/Diphtheria/Tdap
• Rubeola, Mumps, Rubella, Varicella, and Hepatitis B documentation
• Drug screen within 30 days of start
• West Virginia APS/CPS search results
• Kentucky Vulnerable Adult Maltreatment Registry verification
• Orientation Safety Manual Acknowledgement

• Tech Cert - National (Copy)
• State License - Copy of Professional License
• Allied - State License - Hard Copy

Additional Notes

• Meditech EMR
• Flu vaccine declinations are not accepted
• Medical and religious flu exemptions are reviewed case by case
• PAPRs cannot be accommodated due to facial hair
• Latex allergy accommodations are available, with notification required before start
