Job Description

• Specialty: Home Health Nursing
• EMR Experience Required: Home Care Home Base (HCHB)
• OASIS Experience Required
• Minimum 1 year recent nursing experience in specialty
• Average patient load: 5-7 patients per day
• Productivity requirement: Minimum 30 points weekly
• Guaranteed 40 hours per week
• Patient population: Geriatric cases
• No weekend shifts or on-call duties
• Work territory: Up to 60-mile radius from office/hub
• Locals accepted at travel pay rate
• Dress code: Scrubs (any color), no jeans
• Use of iPad provided; cell phone use not allowed during work
• Mileage reimbursed at IRS standard rate
• Timely communication and documentation required

Required Qualifications

• Valid Registered Nurse (RN) license in Alaska
• Minimum 1 year of home health nursing experience within the last year
• Basic Life Support (BLS) certification issued by American Heart Association (AHA) or American Red Cross (ARC)
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience with Homecare Homebase (HCHB) EMR system
• Experience with OASIS documentation

Compliance & Background Requirements

• Background checks within 90 days of start, including:

  • SSN Trace
  • County searches (last 7 years)
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records/Driver’s Record
  • Education verification
  • Work history verification (7 years)
• EPLS/GSA/SAM check within 90 days of start
• HHS/OIG check within 90 days of start
• Fingerprinting per Alaska DHSS requirements (registration and eligible results through Alaska Background Check System)
• State-specific criminal history and employability status checks as applicable
• Physical exam within last 12 months (physician statement)
• Fit test for N95 mask (no specific model required)
• Negative TB test or chest X-ray within 1 year (or TB questionnaire if applicable)
• Immunizations or titers for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug screening: 9-panel drug test within last 12 months
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required
• Signed orientation safety manual acknowledgment (updated annually)
• Competency exams as applicable (medication/pharmacology)

Additional Requirements

• 2 professional references within 2 years for travel/allied/per diem acute care roles
• Full 7 years of work history documented with no gaps
• Annual skills checklist completion
• Compliance with agency background attestation and acceptable use policies
• Proof of current auto insurance required due to driving responsibilities
