Job Opportunity: Licensed Masters of Social Worker (LMSW) – Hospice

Schedule

• Monday to Friday
• Day shift: 8:00 AM – 4:30 PM (5 days/week, 8 hours/day)
• No weekends or on-call required

Patient Load

• Average 5-7 patients per day (geriatric cases)

Required Qualifications

• Valid LMSW (Licensed Master Social Worker) license in New Hampshire
• Minimum 1 year of recent LMSW experience, preferably within the last year
• Basic Life Support (BLS) certification
• Experience with Homecare Homebase (HCHB) electronic medical record system

Additional Requirements

• First-time travelers not accepted (prior travel experience required)
• Locals accepted at travel rate
• Cell phone use not allowed; iPad provided
• Mileage reimbursed at IRS standard rate

Compliance and Background Checks

• Background check required within 90 days of start and updated annually, including:

  • SSN trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide sex offender registry
  • Motor vehicle records
  • Education and work history verification
  • State-specific searches (including New Hampshire state police search)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting may be required depending on placement state (not applicable for NH)
• Education verification of highest degree related to profession
• Employment history verification for 7 years with no gaps
• Two professional references within 2 years of start

Health and Safety Requirements

• Physical exam within last 12 months (not required annually)
• Negative TB test or chest X-ray within required timeframes
• Annual TB questionnaire
• Immunizations or titers for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within last 12 months
• Flu seasonal vaccination or facility-specific declination form (required during flu season)
• COVID vaccination or facility-specific declination

Facility-Specific Documents

• Acceptable Use Policy must be completed prior to start
• Agency background attestation required before start
• Proof of auto insurance if driving is part of job responsibilities (not collected by facility)

This position is ideal for experienced hospice social workers who have prior travel contract experience and are licensed in New Hampshire. The role requires strong documentation skills, flexibility with travel within the territory, and compliance with comprehensive background and health screening protocols.
