Job Opportunity: RN Case Manager - Home Health

Location: White River Junction, Vermont (also serving Bradford, Randolph, Springfield VT, and Lebanon NH areas)
Employment Type: Contract
Contract Length: 16 weeks

Job Description

As an RN Case Manager in Home Health, you will:

• Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
• Coordinate care plans through interdisciplinary communication and case conferences.
• Provide ongoing assessment of patients’ clinical, functional, and emotional status, communicating significant changes to physicians and team members.
• Document evaluations, treatment goals, and objectives.
• Teach patients and caregivers self-care techniques, medication, diet, and provide health counseling as appropriate.

You will work with broad independence while being part of a cohesive patient-care team, supporting a good work/life balance with flexible scheduling.

Qualifications

• Licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• Minimum 2 years of experience in Home Health nursing.

• Prior travel experience is not specified as required.
• This role involves home health and hospice care services across multiple towns in VT and NH.
