Licensed Practical Nurse (LPN) – Long Term Care / Skilled Nursing Facility

Location: Canby, Minnesota
Contract Length: 13 weeks (approximately 3 months)
Shift: Nights (1800-0630)
Shift Length: Primarily 12-hour shifts with one 8-hour shift
Weekend Coverage: Every other weekend
Holiday Coverage: Every other holiday
Hours: 36 hours per week (44 hours one week, 36 hours the next; 80 hours per pay period)
Overtime: Available

Job Details

• Facility size: 43 beds, two-story building
• Patient population: Primarily dementia patients
• EMR system: Matrix and paper flowsheets
• IV care: Not required for LPNs (no IV responsibilities)
• No on-call or call-back duties available or billable

Required Qualifications

• Valid LPN / LVN license in Minnesota
• Minimum of 1 year LPN / LVN experience, including experience within the last year
• BLS (Basic Life Support) certification
• Experience in Long Term Care

Additional Notes

• Local rates apply within 80 miles
• Backouts or quits with less than 14 days’ notice incur a $500 fee
• Agency must have active Minnesota SNSA registration

Summary

This is a night shift LPN position in a long-term care/skilled nursing facility caring primarily for dementia patients. The role involves 12-hour shifts with weekend and holiday rotations every other week. No IV care is required. Candidates must hold an active Minnesota LPN license and BLS certification. Overtime is available, and the assignment length is approximately 13 weeks.
