Facility Overview

• 50-bed inpatient acute rehabilitation hospital
• Focus on treatment and recovery for patients with stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury
• Provides customized rehabilitation programs to maximize health, function, and quality of life

Position Details

• Role: Lead Case Manager (RN)
• Specialty: Case Management
• Shift: 8 AM – 4 PM
• Weekly Hours: 40 hours guaranteed
• Call/Weekend Requirements: None
• Patient Population: General inpatient rehabilitation

Required Qualifications

• Current licensure as a Registered Nurse or Social Worker, or a Bachelor’s/Master’s degree in Social Work or healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist
• Certification in Case Management or Rehabilitation preferred (e.g., CCM, ARN, ACM, ANCC RN-BC)
• Minimum 2 years of social work or case management experience in an inpatient setting (acute/rehabilitation hospital experience highly preferred)
• Previous leadership or supervisory experience strongly preferred
• Basic computer skills and effective communication in English (verbal and written)
• Must be cooperative and a team player
