Job Summary

Provide safe, competent, quality nursing care to patients on a Med Surg/Rehab unit, applying nursing theory, research, and evidence-based practice. Collaborate with interdisciplinary teams to develop and implement patient care plans.

Key Responsibilities

Patient Care

• Deliver patient care using Relationship Centered Care principles and nursing process.
• Assess and evaluate patient needs; apply critical thinking in care management.
• Integrate assessment and intervention skills in nursing care delivery.
• Collaborate with healthcare team on care planning and evaluation.
• Prioritize patient care including teaching, rounding, coaching, discharge planning, and delegation.
• Educate patients and caregivers on care plans, health promotion, and disease prevention.
• Communicate professionally with patients, families, and care team members.
• Manage assignments within Modified Primary Nursing care delivery model.

Patient Experience

• Role model Relationship Centered Care practices such as bedside shift reports, purposeful rounding, medication review, and discharge wrap-up.
• Respect patient values, preferences, cultural diversity, and expressed needs.
• Establish therapeutic relationships with patients and families.
• Use evidence-based practices to understand patient perceptions of care.
• Address patient/family concerns and escalate as needed.
• Demonstrate caring, respect, empathy, and active listening.

Quality and Safety

• Practice safe, patient-centered care and identify barriers to quality.
• Demonstrate clinical skills essential to patient safety.
• Participate in quality improvement projects and evidence-based practice activities.
• Follow institutional protocols to avoid high-risk behaviors and ensure safety.
• Document nursing practice accurately to support quality initiatives.

Operations

• Perform charge nurse duties as assigned.
• Coordinate supply and inventory management with nursing leadership.
• Escalate resource deficits appropriately.
• Use patient-focused, cost-effective approaches to care and resource utilization.
• Facilitate patient throughput during shifts.
• Maintain accurate documentation in compliance with regulatory standards.

Qualifications & Skills

• Current NYS Registered Nurse (RN) License
• BLS Certification
• CRRN Certification preferred for Med/Surg Rehab
• Minimum 2 years of rehab and med/surg nursing experience
• Minimum 2 years RN experience overall
• Travel nursing experience preferred for travel positions (not required for per diem)
• EPIC electronic health record experience highly preferred

Unit Information

• Average Unit Census: 18 patients
• Unit Beds: 24
• Staffing Ratio: 1 nurse to 6 patients
• Schedule: 3 shifts of 12 hours each, weekends required
