Physical Therapist - Contract Position

Job Description

Seeking a qualified Physical Therapist to provide home health therapy services in assigned county territories including Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington, and either Oakland/Berkeley/Alameda area or Vallejo/Benicia/Napa area. Additional travel outside these areas and weekend coverage may be required.

• Must be willing to travel outside the assigned area or work weekends to maintain weekly visit expectations.
• Productivity expectations: Complete at least 27 points per week (points correspond to visit types and hours).
• Visits include OASIS ReCertification, Routine, Evaluation, Discharge, and other related therapy visits.
• Hours approved on a per-visit basis; time includes travel, visit, documentation, and communication with patients and doctors.
• May be required to see COVID+ and COVID PUI patients.

Required Qualifications

• Valid Physical Therapist license in California
• Minimum of 1 year of Physical Therapist experience, including experience within the last year
• Advanced Cardiovascular Life Support (ACLS) certification
• Basic Life Support (BLS) certification
• Pediatric Advanced Life Support (PALS) certification
• Neonatal Resuscitation Program (NRP) certification
• Valid Driver’s License, Auto Insurance, and Auto Registration
• Up-to-date immunizations and screenings including:

  • TB (annual)
  • MMR
  • Varicella
  • Hepatitis B
  • Tdap (within 10 years)
  • Annual Flu vaccine
• Drug screen and physical exam clearance

Additional Information

• Agencies must ensure contract dates and requested time off are disclosed and match exactly as confirmed; failure to comply may result in removal from vendor set.
• Prior travel experience is not explicitly required.
