Physical Therapist (Home Health)

Location: Cheyenne, Wyoming
Contract Length: 13 weeks

Job Overview

Provide home-based physical therapy services across three counties, treating 4-8 patients per day depending on location. Patient diagnoses commonly include CHF, COPD, total hips, knees, shoulders, strokes, and dementia.

Work Environment & Schedule

• Flexible scheduling with self-scheduling of patient visits
• No weekend or holiday requirements (flexible time off options available)
• Orientation: 3 days in the field with a preceptor, working alongside different PTs and PTAs
• Dress code: Ceil blue scrubs (not provided)

Responsibilities

• Deliver home health physical therapy services
• Supervise Physical Therapist Assistants (PTAs)
• Preferred experience in hand therapy and geriatric care
• Complete OASIS-E standardized assessments for all Start of Care, Resumption of Care, and Recertifications every 60 days
• Use Epic software and remote client systems

Required Qualifications

• Licensed Physical Therapist in Wyoming (license must be obtained before applying)
• BLS Certification
• Experience with home health patient care, including common diagnoses listed above

• Permanent address must be at least 25 miles from the facility location
• Housing options available, including accommodations for 40ft campers (note seasonal water shutoff)
