Minimum Requirements
<ul><li>Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA</li><li>Active State License is Required to Start the Assignment</li><li>BLS Certification May Be Required from AHA or ARC</li><li>Minimum 1 year home health experience</li><li>Current Physical Therapy licensure in the state of Washington or compact license granting privilege to practice in the state</li><li>Current CPR certification</li><li>Current driver's license, vehicle insurance, and access to a dependable vehicle</li></ul>
 
Assignment Details
<ul><li>40 hours per week.</li><li>Shift hours: 8:00am - 4:30pm.</li><li>Shift days: Monday through Friday.</li><li>No on-call requirement.</li><li>Weekend rotation required.</li><li>Coverage area includes King County and Snohomish County.</li><li>May float to other nearby agencies as needed.</li><li>Mileage is reimbursed between visits, from first patient to last patient.</li><li>Productivity expectation: minimum 31 points.</li></ul>
