Job Opportunity: RN - Case Management (Travel)
Location: Philadelphia, Pennsylvania
Employment Type: Travel Contract
Contract Length: 13 weeks
Job Description
- Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
- Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
- Enhance quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness.
- Integrate case management, utilization review, and discharge planning functions.
- Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
- Collaborate strongly with interdisciplinary teams (IDT).
Work Schedule
- Monday to Friday, 8:00 AM to 4:30 PM
Required Qualifications
- Valid RN license in Pennsylvania
- Minimum 2 years of RN Case Management experience, including experience within the last year
- Experience in Acute Care and Long Term Acute Care (LTAC) settings
- Associate of Science in Nursing or Associate Degree in Nursing
- Current BLS certification issued by the American Heart Association
