Physical Therapist - Home Health (Travel/Contract)

Location: Concord, California
Contract Length: 13 weeks

Job Details

• Specialty: Home Health Physical Therapy
• Shift: Day shift, Monday to Friday (5 days x 8 hours) from 8:00 AM to 5:00 PM
• Hours per week: 40
• Average caseload: 5-7 patients per day
• Patient population: Geriatric cases
• Territory: Up to a 60-mile radius from the office hub (Far East area: Discovery Bay to Pittsburg)
• Dress code: Scrubs of any color; jeans are not allowed
• Equipment provided: iPad
• Mileage reimbursed at IRS standard rate
• No on-call or weekend work required
• Locals accepted at travel rate
• Flexibility with territory expected

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification
• Experience with Home Care Home Base (HCHB) Electronic Medical Record system

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID)
• Emergency contact form prior to start
• Background checks within 30 days including federal, county, sex offender, motor vehicle record, and FACIS Level 3
• Annual EPLS/GSA/SAM and HHS/OIG checks
• Education verification (third-party or acceptable in-house verification)
• Physical exam within 6 months prior to start
• Negative TB test (PPD, Quantiferon Gold, or T-spot) within 6 months; chest x-ray acceptable within 5 years if history of positive TB
• Immunization or titer proof for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (vaccine series or declination)
  • Varicella (titer, vaccine, or history of chickenpox)
• Drug screening: 10-panel within 30 days of start (instant acceptable)
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
• Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years)
• Work history verification for last 3 employers
• Annual skills checklist and competency exams as applicable

Additional Notes

• First-time travel therapists are accepted
• Cell phone use is not allowed during work
• Must be flexible with travel territory within the designated radius
