Job Opportunity: Speech-Language Pathologist (Home Health)
Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: 16 weeks (approximately 4 months)
Job Overview
- Specialty: Speech-Language Pathology in Home Health setting
- Patient Focus: Home Health, Wound Care, Geriatric Home Health Patients
- Visits: Minimum 23-24 visits per week required to qualify for a 40-hour guarantee
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 5:00 PM)
- Weekend Work: Not required
- Territory: May include surrounding counties; specifics discussed during interview
- Locals: Accepted within a 50-mile radius
Required Qualifications
- Valid Speech Language Pathologist license in California
- Minimum 1 year of Speech Language Pathologist experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification

Compliance and Documentation Requirements
Candidates must provide the following prior to starting:
- Government-issued photo ID (Driver’s License or State ID)
- Emergency Contact Form
- Background checks within 1 year, including SS Trace, County searches (7 years), Nationwide, and Sex Offender registry
- Annual EPLS/GSA/SAM and HHS/OIG checks
- Certifications: BLS (required), ACLS and PALS preferred but not mandatory for this role
- Physical exam within 6 months signed by MD/DO, including statement of no active communicable diseases
- Immunizations and titers:
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Negative TB test or chest X-ray within 1 year
- Measles, Mumps, Rubella (proof of immunity or vaccination)
- Hepatitis B (titer, vaccination series, or declination)
- Varicella (titer, vaccination, history of chickenpox, or declination)
- Drug screening: 9-panel drug screen within 1 year (instant acceptable)
- Seasonal flu vaccination or declination form during flu season (Oct 1 - Mar 31)
- COVID-19 vaccination or approved exemption form
- Auto insurance card (if applicable)
- References: 2 professional references within 3 years (for travel, allied, per diem acute care roles)
Additional Information
- Mileage reimbursed at IRS standard rate from first to last patient visit (not to/from home)
- Free residential parking available
- No on-call requirement; on-call duties are discretionary
- Contract length is non-negotiable: less experienced clinicians must commit to 6 months
- Emphasis on qualities such as patient-centered care, punctuality, self-motivation, and timely charting completion
