LVN / LPN Travel/Contract Position

Location: Dayton, Ohio
Contract Length: 13 weeks

Job Overview

• Work 36 or 48 hours per week (OT paid after 48 hours for 48-hour contracts)
• Unit has 20-21 beds with a patient ratio of 1:7
• Patients require less acute care, similar to a nursing home setting
• No telemetry monitoring, blood products, or Bipap unless long term
• Unit may use a team nursing model (RNs, LPNs, PCTs partner to care for patient groups) or primary nursing depending on unit needs
• Patients are long length of stay, awaiting guardianship, placement, or insurance; require custodial care and cannot be discharged from the ED for safety reasons
• Navy blue scrubs required

Requirements

• Minimum 1.5 years nursing experience; experience in level 1 or 2 trauma centers preferred
• Travel nursing experience preferred
• EPIC experience required
• Must pass an online EKG telemetry competency assessment before starting
• BLS certification required; must be able to read and interpret EKG strips
• Nurses will be fitted for N95 masks prior to working on the unit unless PAPR is needed/offered
• Must be available to work every other weekend and holidays
• Floating required: may float to other specialty clusters or campuses within 15 miles, including high-risk respiratory/COVID units
• Premier Health may cancel up to 2 shifts per 13-week contract due to low census
• Premier Health may cancel contract with 2 weeks' notice without penalty
• Travelers are not eligible for Premier Health employee incentives

Documentation & Compliance Requirements

• I-9 and E-Verify (Lotus branded forms required)
• Government-issued photo ID
• Background checks including 7-year criminal check, EPLS/GSA/SAM, HHS/OIG
• Fingerprinting only required for select units (not required for this unit)
• Education verification (Primary source)
• Licensure verification (Ohio license, NURSYS verification)
• Physical exam within 2 years indicating fitness to work
• Negative TB test or chest x-ray; annual TB questionnaire if past positive
• Required vaccinations or immunity documentation: Rubeola, Mumps, Rubella, Varicella, Hepatitis B (vaccination series, titer, or declination)
• Drug screening: 9 or 10 panel negative drug screen within 12 months, including marijuana panel
• Flu vaccination or approved medical/religious exemption required during flu season
• Respiratory safety clearance and medical questionnaire for respirator protection
• References: 2 professional references within 2 years (1 must be manager/supervisor/charge RN)
• Current resume/application
• Skills checklist updated within 1 year
• Competency exams including medication/pharmacology, unit-specific, and dysrhythmia/EKG competency (must pass with 80% or higher; up to 3 attempts allowed)

Additional Notes

• Request Return to Office (RTO) approval must be submitted with initial application; no RTO added after offer
• Floating assignments will be medically appropriate and may include support on high-risk respiratory or COVID units
• Premier Health reserves rights to cancel shifts or contracts as needed

This position is ideal for LVNs/LPNs with some acute care experience, EPIC proficiency, and flexibility to float and work varied shifts in a long-term care style hospital unit.
