Job Description

• Position: Physical Therapist (Field/Home Health)
• Work Locations: Hayward / Fremont area
• Schedule Options: Tuesday-Saturday or Wednesday-Sunday
• Home Health experience preferred but candidates without experience will be considered
• California license preferred; candidates with pending CA license who have applied will be accepted
• OASIS experience required
• Guaranteed hours based on completed visits (25 visits = 40 hours)
• COVID exemptions permitted
• Mileage reimbursed at IRS standard rates

Required Credentials & Documentation

Licenses & Certifications

• California Physical Therapy License (preferred; pending applications accepted)
• OASIS experience required
• ACLS (AHA Instructor or Provider) – as applicable
• BLS (AHA Instructor or Provider) – required for all
• PALS, NRP, EFM – as applicable
• National Tech Certification Verification – as applicable
• Primary source verification of state license
• NURSYS verification (if applicable)

Background Checks (Annual)

• Standard 7-year background check including SS trace and county searches
• EPLS/GSA/SAM check
• HHS/OIG check

Health & Immunization Requirements

• Physical exam within 12 months of start
• TDAP vaccine within past 10 years (no declinations accepted)
• Negative TB test or chest X-ray within 1 year (with protocols for past positives)
• Measles, Mumps, Rubella (MMR) proof or titer for candidates born in 1957 or later (no declinations accepted)
• Hepatitis B titer or vaccine series (declination accepted if within 1 year)
• Varicella immunity proof (titer, vaccines, or history of chickenpox; no declinations accepted)
• Negative 9-panel drug screen within 1 year
• Seasonal flu vaccination (declinations accepted)
• COVID-19 vaccination or approved medical/religious exemption required prior to start, including booster dose

• Government-issued photo ID (Driver’s License or State ID)
• Valid passport copy before start
• Passport-style photo for badge
• Personal or auto insurance with current expiration dates
• Auto registration with current expiration dates
• Signed acknowledgments for training content and hand hygiene policy
• Two professional references
• Annual skills checklist
• Medication/pharmacology competency exam (as applicable, minimum 80% score)
• Competency exam (as applicable, minimum 80% or Angoff score)

• Hours are guaranteed if 25 or more visits are completed weekly.
• Mileage reimbursement follows IRS standard rates.

• Candidates with pending CA licenses must have applied prior to submission.

This position is ideal for physical therapists with home health experience or those willing to gain experience in a travel/contract role within California.
