Job Opportunity: Therapy/Rehabilitation (Travel/Contract)

Location: San Jose, California


Position Details

  • Specialties Needed:
    • Physical Therapists (PTs) in various counties including Sonoma, Marin, Contra Costa, Alameda, Santa Clara, San Joaquin/Stanilaus, Sacramento
    • Speech-Language Pathologists (SLPs) in Alameda, San Joaquin/Stanilaus, Santa Clara, Sonoma/Marin counties
  • Patient Load: Focus on patients per week rather than per day
  • Visits Required: Minimum 23-24 visits per week to qualify for a 40-hour guarantee
  • Types of Cases: Home Health, Wound Care, Geriatric Home Health Patients
  • Experience Required:
    • Minimum 1 year in Home Health
    • 2 years overall clinical experience
  • First-Time Travelers: Accepted
  • Weekend Requirement: No weekends required
  • On-Call: Not required (discretionary)
  • Territory: May include surrounding counties; discussed during interview
  • Locals Accepted: Yes, within a 50-mile radius
  • Pending License: Not accepted; must have valid state license at submission
  • Dress Code: Any color scrubs, closed-toe shoes
  • Charting System: Kinnser/Wellsky; charting must be completed onsite, not at home
  • Parking: Free residential parking available
  • COVID & Flu Policy:
    • COVID vaccination required; religious declination allowed
    • Flu vaccination optional; declination required if declined
  • Contract Length: 4-6 months (less experienced clinicians must commit to 6 months, non-negotiable)

Required Certifications & Licenses

  • Basic Life Support (BLS) certification required at time of submission
  • State License valid and in hand at time of submission
  • Advanced certifications such as ACLS and PALS preferred but not explicitly required for all
  • National tech certifications as applicable

Compliance & Documentation Requirements

  • All required documents must be submitted and approved before starting patient care (compliance process may take 2-3 weeks)
  • Required documents include but are not limited to:
    • Government-issued photo ID (Driver’s License/State ID)
    • Emergency Contact Form
    • Background Checks (7-year search including SS trace, county searches, sex offender registry)
    • Annual EPLS/GSA/SAM and HHS/OIG checks
    • Physical exam within 6 months signed by MD/DO, confirming no active communicable diseases
    • Immunizations and titers: Tetanus/Diphtheria/Tdap, Measles, Mumps, Rubella, Hepatitis B, Varicella
    • Negative TB test or chest X-ray within 1 year
    • Drug screening (9-panel) within 1 year
    • Seasonal flu and COVID vaccination documentation or approved declinations
    • Auto insurance card (for travel clinicians)
    • References: 2 professional references for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years)
    • Annual skills checklist and competency exams as applicable
