Physical Therapy Assistant - Home Health (Travel/Contract)

Location: Tallahassee, Florida
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40


Job Overview

  • Provide physical therapy assistance in home health settings with both home visits and periodic office meetings.
  • Patient load: 5-10 patients per day, averaging 5-6 patients daily.
  • Types of cases: Start of Care and Follow-Ups.
  • Travel required up to 75 miles from home clinic, with approximately 250-300 miles traveled weekly.
  • Visits include patients in care facilities and private homes.

Required Qualifications

  • Valid Physical Therapy Assistant license in Florida.
  • Minimum of 1 year of recent Physical Therapy Assistant experience within the last year.
  • Compliance with applicable state and federal regulations.

Schedule & On-Call

  • Standard schedule is Monday through Friday, 8 AM to 5 PM.
  • Some positions may have alternate schedules (e.g., 4x10 shifts Friday-Monday) depending on location.
  • On-call requirements vary by location; discuss during interview.
  • Weekend work varies by location and profession; discuss during interview.

Productivity Expectations

  • Weekly productivity units expected: approximately 25-32 units.

Systems Used

  • Electronic Medical Records (EMR): HomeCare HomeBase.
  • Communication tools: Outlook, Microsoft Teams.

Travel & Mileage

  • Mileage reimbursed at IRS standard rate.
  • Compensation covers travel from patient home to patient home only; commute to/from first and last patient is not billable.
  • Local candidates accepted at the same rate.

Compliance & Documentation Requirements

Candidates must complete and maintain the following prior to and during assignment:

Background & Verification

  • Standard 7-year background check including SS trace, county, nationwide, sex offender, and state-specific searches.
  • EPLS/GSA/SAM and HHS/OIG checks within 14 days of start, updated monthly.
  • Education verification for applicable degree.
  • Work history verification for at least two most recent employers or last 7 years.
  • Two professional references required.

Fingerprinting

  • State-specific fingerprinting requirements apply (e.g., Florida Level II background screening).
  • Fingerprinting results must be current and completed prior to start.

Health & Immunizations

  • Physical exam within 12 months confirming good health and no communicable diseases; updated annually.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Proof of immunity or vaccination for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
  • 10-panel drug screen per Aya standard.
  • Seasonal flu vaccination or documented declination.
  • COVID-19 vaccination series or documented declination.

Facility-Specific Documents & Training

  • Review and sign confidentiality, privacy policies, HIPAA, compliance training, code of conduct, and other facility-required documents prior to start.
  • Complete home health-specific training and policy acknowledgments.
  • Annual re-signing of supplier packets required after any break in service or after one year of consecutive work.

Additional Notes

  • No guaranteed hours; if patient census is low, contractor may be floated to nearby clinics to pick up hours.
  • Must be willing to discuss on-call and weekend requirements during interview.
  • Candidate must be prepared to comply with all state-specific and facility-specific credentialing and compliance requirements.
