Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies
- Supervise and evaluate Home Health Aides and other LVNs/LVNs
- Conduct home visits with an average of 5-8 patients per day
- Types of cases include Start of Care, Wound Care, and Follow-up Visits
- Must have reliable transportation and current driver’s license
- Willingness to travel up to 75 miles from home clinic; approximately 400 miles traveled weekly
- Use of HomeCare HomeBase (HCHB) electronic medical record system required, with recent proficiency (within last 6 months) or willingness to retrain
Required Qualifications
- Valid LPN/LVN license in Maine
- Minimum 1 year of recent Home Health experience within the last year
- BLS certification issued by the American Heart Association or American Red Cross
- Experience working in a Home Health setting
- Proficient with HomeCare HomeBase (HCHB) electronic medical record system
Experience
- Recent specialty experience required; first-time travelers not accepted
- Prior travel experience is not explicitly required
Productivity Expectations
- 32 weekly units of productivity expected

Documentation and Compliance Requirements
Candidates must complete and maintain the following prior to and during assignment:
Background and Verification
- Standard background check covering last 7 years of residence/employment/education
- Education verification for applicable degree
- Sex offender search and FACIS screening (for applicable states)
- State-specific background and registry checks as required by assignment location
- Work history verification for at least two most recent employers or last 7 years
Health and Immunizations
- Physical exam within 12 months confirming good health and no communicable disease symptoms
- Negative TB test or chest X-ray within 12 months (no pending results)
- Proof of immunity or vaccination for Measles (Rubeola), Mumps, Rubella (MMR), Hepatitis B, Varicella, and COVID-19 (or documented declination)
- Seasonal flu vaccination or declination documentation for current season
- 10-panel drug screen per standard requirements
Certifications and Training
- BLS certification current at start
- Review and sign compliance documents including HIPAA, confidentiality, privacy policies, and code of conduct
- Complete home health-specific training and quizzes prior to start
- Sign all required agreements and attestations, including supplier packets at assignment start and annually if working over one year consecutively
Additional Requirements
- Personal or auto insurance policy in candidate’s name or listing candidate as a driver, current and valid
- Must be willing to comply with all state-specific fingerprinting and background check requirements prior to start
Travel and Local Candidates
- Local candidates accepted at the same rate
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed)
