Job Opportunity: Medical Surgical LVN / LPN

Job Description
Seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a medical surgical travel assignment. The role involves providing nursing care in an acute or ambulatory care setting with mandatory floating to other units as needed.
Schedule & Work Details
- Guaranteed 40 hours per week
- Weekend Requirement: Every other weekend (2 weekends per month)
- Return to Office (RTO): Maximum 5 days, no more than 2 consecutive days; no RTO on holidays or weekends
- Mandatory float within scope of practice to meet facility needs
Required Qualifications
- Valid LPN / LVN license in Illinois
- Minimum 1 year of medical-surgical nursing experience within the last year
- BLS certification issued by the American Heart Association
Certifications & Competencies
- Medication Test/Pharmacology Competency Exam (required within 12 months of start)
- Basic Life Support (BLS) certification (AHA only)
- Other certifications may be required depending on unit assignment
Health & Compliance Requirements
- Physical exam within 12 months prior to start, updated annually
- Fit mask test (N95 or equivalent) within 1 year, required annually
- Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination
- Negative TB test (QGold or TSPOT) within 12 months, updated annually
- Immunizations or titers for Measles, Mumps, Rubella, Varicella, and Hepatitis B as per facility policy
- Negative 9-panel drug screen within 90 days of start, updated annually
- Seasonal flu vaccination or approved medical/religious exemption during flu season (Oct 1 - Apr 1)
Background & Documentation
- Background check within 12 months covering all locations lived/worked/schooled in past 7 years, updated annually
- Various state and federal screenings as required
- Government-issued photo ID and Social Security card required
- Completion of facility-specific forms and compliance documents prior to start

- This is a correctional facility setting
