Job Description

Seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a medical surgical travel assignment. The role involves providing nursing care in an acute or ambulatory care setting with mandatory floating to other units as needed.

Schedule & Work Details

• Guaranteed 40 hours per week
• Weekend Requirement: Every other weekend (2 weekends per month)
• Return to Office (RTO): Maximum 5 days, no more than 2 consecutive days; no RTO on holidays or weekends
• Mandatory float within scope of practice to meet facility needs

Required Qualifications

• Valid LPN / LVN license in Illinois
• Minimum 1 year of medical-surgical nursing experience within the last year
• BLS certification issued by the American Heart Association

Certifications & Competencies

• Medication Test/Pharmacology Competency Exam (required within 12 months of start)
• Basic Life Support (BLS) certification (AHA only)
• Other certifications may be required depending on unit assignment

Health & Compliance Requirements

• Physical exam within 12 months prior to start, updated annually
• Fit mask test (N95 or equivalent) within 1 year, required annually
• Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination
• Negative TB test (QGold or TSPOT) within 12 months, updated annually
• Immunizations or titers for Measles, Mumps, Rubella, Varicella, and Hepatitis B as per facility policy
• Negative 9-panel drug screen within 90 days of start, updated annually
• Seasonal flu vaccination or approved medical/religious exemption during flu season (Oct 1 - Apr 1)

Background & Documentation

• Background check within 12 months covering all locations lived/worked/schooled in past 7 years, updated annually
• Various state and federal screenings as required
• Government-issued photo ID and Social Security card required
• Completion of facility-specific forms and compliance documents prior to start
