- Varicella: Proof of immunity or two doses of vaccine; no declinations accepted
- Tdap: Valid within 10 years; no declinations accepted
- TB Screening: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire
- Fit Test: Required annually (specific mask types for WI and MI)
- Ascension 12 Panel Drug Screen: Completed within 30 days prior to start and updated annually

- Health Statement or physical exam completed prior to first assignment and updated annually

- Completion of Ascension-specific onboarding modules and attestations prior to or at start
- Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 30 days of assignment start; updated annually
- Employment and education verifications required prior to start and updated annually
- Professional references: At least two qualifying references required prior to first assignment
- Ascension Cerner Test-Out and other facility-specific competency tests required prior to start
