  • Tdap: Valid within 10 years; no declinations accepted
  • TB Screening: Positive IGRA or PPD results, negative chest X-ray within 1 year, and annual TB questionnaire

• Employment and education verifications required prior to start and updated annually
• Professional references: At least two qualifying references required prior to first assignment
• Ascension Cerner Test-Out and other facility-specific competency tests required prior to start
