Job Opportunity: CT Technologist

Location: Maywood, Illinois
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)
Schedule: Includes Every Other Week (EOW) rotation

Required Credentials & Certifications

• BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel
• ARRT Certification
• State of Illinois License
• License/Certificate Verification (must be dated within 30 days of start)
• Fit Test (personal health requirement)
• Trinity 10 Panel Drug Screen (completed within 30 days of start; includes specific drug panel, no THC)
• COVID-19 Vaccine (proof of booster, declination not accepted in IL)
• MMR Vaccination (proof of 2 vaccinations or IgG titer)
• Hepatitis B Vaccination (proof of vaccination series required; titers not accepted)
• Influenza Vaccine (proof required for flu season; declination not accepted in IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) (TB screening)
• Varicella Vaccination (proof of 1 vaccination or IgG titer)
• Physical/Health Statement (exam within last year certifying fitness to work)

Professional & Compliance Requirements

• Trinity Code of Conduct Form
• Trinity I9 Verification
• Confidentiality of Patient & Proprietary Information and System Security Agreement
• Integrity and Audit Services Form
• Motor Vehicle Report (MVR) (if applicable, dated within 30 days)
• Trinity Profile (resume, skills checklist within a year, 2 professional references)
• Conflict of Interest Disclosure Form
• Criminal Background Check (federal, nationwide, county for last 7 years, SSN verification, dated within 30 days)
• Education Verification (highest academic degree or diploma/GED)
• Exclusion Lists Checks: EPLS, OIG/GSA, NSO (all dated within 30 days)
• Joint Commission Testing (CORE Exam) (within last year)
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
