Surgical Technician - Grand Rapids, Michigan (LTA Contract)

• Shift: 08:45 AM – 05:15 PM, 5 shifts per week
• On-Call: Saturday/Sunday 7 AM–7 PM or 7 PM–7 AM; plus 7 PM–7 AM call 4 weekdays per LTA
• Contract Length: 13 weeks

Required Certifications and Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel.
• Certified Surgical Technologist (CST): Required.
• License/Certificate Verification: Must be dated within 30 days of start.

Health and Safety Requirements

• Fit Test: Required.
• Trinity 10 Panel Drug Screen: Must be completed within 30 days of start (preferred within 14 days). Includes testing for cocaine, amphetamines, opiates, PCP, barbiturates, benzodiazepines, methaqualone, methadone, and others. No THC allowed.
• COVID-19 Vaccine: Proof of booster by September 2025, or declination/exemption with agency letterhead (declination not accepted in CA or IL).
• Influenza Vaccine: Proof of vaccine, declination, or exemption for flu season 10/1/2024–4/30/2025. Declination not accepted in CA or IL.
• MMR: Proof of 2 vaccinations or IgG titer lab report.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Varicella: Proof of 1 vaccination or IgG titer lab report.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year for Michigan.
• Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.

Professional and Compliance Requirements

• Trinity Specialty Specific Competency Testing: Specialty exam completed within the last year.
• Trinity Skills Checklist: Provided by agency for clinical contractors.
• Trinity Code of Conduct Form: Signed form required.
• Trinity I9: Standard I9 completed by agency.
• Confidentiality & Security Agreement: Signed form required.
• Integrity and Audit Services Form: Signed within days prior to start.
• Conflict of Interest Disclosure Form: Signed form required.
• Criminal Background Check: Federal, nationwide, county criminal check from last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification: Highest academic education verified by third party screening.
• EPLS (Excluded Parties List System): Report dated within past 30 days required.
• GSA/OIG Exclusion Check: Report dated within past 30 days required.
• Joint Commission CORE Exam: Within the last year.
• NSO (National Sex Offender) Check: Dated within 30 days of start, including statewide check for Michigan.
• System Access Form: Must be completed and uploaded one week before start.
