- Guaranteed Hours: 72 hours per 2 weeks; may be called off for one 8-hour shift every 2 weeks
Unit Information
- Unit census: 50-70 patients daily
- Staffing: 5-6 SLPs on weekdays, 3-4 on weekends
- All SLPs must complete Modified Barium Swallow Impairment Profile (MBS-IMP)
- Patient population ranges from ICU level to Skilled Nursing Facility (SNF)/Outpatient (OP) level
- ICU rotation not required; clinical readiness will be assessed if ICU coverage is needed

Required Documentation & Compliance
- Background Checks:
- Must be completed within 45 days of start, covering all locations lived/worked in past 7 years
- Includes SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, MVR (if applicable), and international background if applicable
- Returning candidates with less than 30-day gap may not need new background check
- Education Verification: Required for all candidates
- References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment Application: Must include at least last 5 years of employment
- Skills Checklist: Required annually for applicable units
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO
- Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
- Immunizations & Health Requirements:
- BLS certification (AHA or Canadian Heart & Stroke Foundation accepted)
- Tetanus/Diphtheria/Tdap within 10 years or declination
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination (Alaska placements require positive titer and vaccine series)
- Varicella vaccination or positive titer
- Seasonal Flu vaccine or declination during flu season
- COVID vaccine or declination
- Fit Mask test (N95 or equivalent) required for travel candidates, updated annually
- Respiratory Safety Clearance required annually for travel candidates
- Licenses:
- Must hold a valid Alaska license for SLP
- NURSYS verification required if applicable
- Certifications:
- BLS required
- Other certifications (ACLS, PALS, NRP, etc.) as per unit description or clinical track
- Training Modules:
- Completion of Providence WorkRamp modules required prior to start, including core, specialty, and regulatory modules as applicable
- Epic training modules required for charting system proficiency
Additional Notes
- Candidates must comply with Providence’s floating policy acknowledgment (may be required to float within reasonable distance)
- Candidates must submit all required documentation and complete all compliance requirements prior to start
- No on-call duties required for this position
