Job Opportunity: Physical Therapist (PT) - Home Health

Location: San Jose, California
Employment Type: Travel/Contract


Position Details

  • Specialty: Physical Therapy (PT) in Home Health
  • Patient Focus: Home Health, Wound Care, Geriatric Home Health Patients
  • Visits: Minimum 23-24 visits per week to qualify for a 40-hour guarantee
  • Weekend Requirement: No weekends required
  • Locals Accepted: Yes, within a 50-mile radius
  • Dress Code: Any color scrubs, closed-toe shoes
  • Charting System: Kinnser/Wellsky (must be completed onsite, no charting from home)
  • Parking: Free residential parking
  • COVID & Flu Policy: COVID vaccine required (religious declination allowed); flu vaccine declination allowed with form
  • Contract Length: 4-6 months (less experienced clinicians must commit to 6 months, non-negotiable)

Required Qualifications

  • Minimum 2 years overall clinical experience
  • Minimum 1 year experience in Home Health
  • First-time travelers accepted
  • Valid Physical Therapy license for California required at time of submission (no pending licenses accepted)
  • BLS certification required at time of submission
  • No on-call required (discretionary but not mandatory)

Compliance & Documentation Requirements

  • Government-issued photo ID (Driver’s License/State ID)
  • Background checks within 1 year, including SS Trace, County searches, Nationwide Sex Offender, EPLS/GSA/SAM, and HHS/OIG (annual updates required)
  • Certifications: BLS (Basic Life Support), ACLS and PALS preferred (American Heart Association or American Red Cross)
  • Physical exam within 6 months signed by MD/DO, including statement of no active communicable/infectious diseases
  • Immunizations and titers:
    • Tetanus/Diphtheria/Tdap (within 10 years or medical/religious exemption)
    • Negative TB test or chest X-ray within 1 year
    • Measles, Mumps, Rubella (proof of immunity or vaccination per policy)
    • Hepatitis B (titer, declination, or vaccine series)
    • Varicella (titer, vaccination, history of chickenpox, or declination)
  • Drug screening: 9-panel drug screen within 1 year (instant drug screen acceptable)
  • Seasonal flu vaccination or declination form required during flu season (Oct 1 - Mar 31)
  • COVID vaccination or approved medical/religious exemption required
  • Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years)
  • Skills checklist required annually
  • Auto insurance card required

Additional Notes

  • Mileage reimbursed per IRS standard from first to last patient (not to/from home)
  • Territory/County coverage details discussed during interview; may include surrounding counties
  • All required compliance documents must be submitted before starting patient care (compliance processing may take 2-3 weeks)
  • All requested time off (RTO) must be submitted at time of application; no additional RTO accepted after offer
