Job Opportunity: Case Manager (Registered Nurse)

Job Description

• Backfill for holiday coverage
• Inpatient discharge planning
• Float to all inpatient units
• Weekend shifts every other weekend
• Auto offer

Required Experience

• Minimum 3 years acute care experience
• Experience with EPIC, ambulatory care, home health, hospice, SNF (Skilled Nursing Facility), ANU, and LTACH referrals

Certifications & Licenses Required

• Current, unencumbered State RN License
• ACLS (Advanced Cardiac Life Support) - AHA only
• BLS (Basic Life Support) - AHA only
• Other certifications may be preferred depending on unit (e.g., PALS, NRP, CHEMO, TNCC, EFM, MAB, NIH Stroke Certification, CPI)

Health & Compliance Requirements

• Background checks including criminal, sanction, and exclusion screenings within 90 days of start
• Education verification of highest level completed
• Physical exam within 90 days of start, signed by MD, DO, PA, NP, or APRN
• Fit mask test within 90 days of start (specific mask models required)
• Immunizations and titers:

  • Tetanus/Diphtheria/Tdap (within 10 years or signed declination)
  • Negative TB test or chest X-ray updated annually
  • MMR (Measles, Mumps, Rubella) proof of vaccination or positive IgG titers
  • Hepatitis B positive titer or signed declination
  • Varicella proof of vaccination or positive IgG titer
  • Seasonal flu vaccination or signed declination (masking required during flu season)
• Drug testing: Oral saliva drug screen within 30 days of start (specific analytes required)
• Color blind exam within 90 days of start

Documentation & Other Requirements

• Valid photo ID (Driver’s License or Passport)
• Photo for badge (passport style, no filters)
• Employment application with last 7 years of work history
• Work history verification for past 3 years
• Competency exams and medication/pharmacology competency as applicable
• Completion of facility-specific orientation and safety modules on first day
• References as required by agency

• Prior travel experience is not explicitly required for this position.

This position is ideal for experienced acute care RNs with strong discharge planning skills and familiarity with multiple care settings and EPIC documentation.
