Job Opportunity: Occupational Therapist (OT) - Home Health

Location: Tulsa, Oklahoma
Employment Type: Travel/Contract
Contract Length: 13 weeks


Position Details

  • Specialty: Home Health Therapy/Rehabilitation
  • Average Patients per Day: 5-7
  • Guaranteed Hours: 40 hours per week (once productivity is met)
  • Types of Cases: Geriatric
  • Territory: Up to a 60-mile radius from the hub/office
  • Locals Accepted: Yes, at travel rate

Requirements

  • Years of Experience: Minimum 1 year recent experience in specialty

  • Annual TB questionnaire required
  • Immunizations/vaccinations required or documented declinations for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • 9 Panel Drug Screen within last 12 months
  • Flu seasonal vaccination or facility-specific declination form required during flu season
  • COVID vaccination or facility-specific declination required

Documentation & Verification

  • 7 years of full work history with no gaps required on employment application
  • Employment history verification within 90 days of start
  • 2 professional references required (travel, allied, per diem acute care) within 2 years of start
  • Annual skills checklist required

Facility Specific Requirements

  • Proof of current auto insurance required (for positions involving driving)
  • Completion of facility Acceptable Use Policy prior to start
  • Facility-specific forms related to flu and COVID vaccination status may be required
